Forward Sports Care is a cash-based private physical therapy practice based in Madison, WI. FSC was founded by Ted German, DPT, to provide comprehensive personalized PT for endurance athletes. Being cash-based gives us the flexibility, freedom, and time to provide care above and beyond what you will typically experience at an insurance-driven physical therapy appointment.

Ted German, PT, AAS, BA, DPT, CIMT, CPRS, has over 25 years of experience in outpatient orthopedic and sports physical therapy. He has been helping endurance athletes reduce pain and optimize performance since 2007. In addition to his role at Forward Sports Care, Ted works at Group Health Cooperative in Madison, WI with special interests in endurance athletes and spine patients. Ted has been racing bikes since 2000 when he competed in his first triathlon. He finished Ironman Wisconsin in 2006. Formerly active in road racing and time trials, Ted now focuses primarily on racing cyclocross and MTB. Dr. German’s education includes a Doctorate in Physical Therapy from St. Catherine University, Minneapolis, MN and a Bachelor of Arts in Kinesiology from Concordia University, St. Paul, MN. He is a Certified Integrated Manual Therapist, Certified Pilates Rehabilitation Specialist, and is certified in Functional Dry Needling.
The forward Sports care experience
Physical Therapy appointments will begin with a comprehensive sport-specific full body evaluation. This evaluation will determine your course of care. As appropriate, your treatment may include but not limited to soft tissue mobilization, cupping, dry needling, joint mobilization, muscle energy, and myofascial release. Offerings and pricing
PT Bike Fit (2-3 hours): $350 includes PT evaluation and treatment described above plus a complete bike fit.
PT appointments (1 hour): $175 described above
Bike fit only (1-2 hours): $195 for any bike without aerobars; $225 for any bike with aerobars
